Things You'll Need

  • Marriage license

  • Social Security card (with your old name)

  • Photo ID (with your old name)

Instructions

      Visit socialsecurity.gov to download and print the Application for a Social Security Card--Form SS-5. You can also visit any Social Security office or call Social Security toll-free at (800) 772-1213 to request an application.

      Complete the application and present proof of your identity and legal name change. Original documents with both your new name and your old name, such as your marriage license, your old Social Security card, and a photo ID with your old name, will be used to verify your identity.

      Use alternative forms of identification if necessary. For example, if you can't find your old Social Security card, provide your original birth certificate, passport, driver's license, citizenship/naturalization certificates or immigrant visa instead.

      Take or mail your application and the supporting documents to your local Social Security office- visit socialsecurity.gov/locator to find the nearest office.   • If you live in the New York City metropolitan area- Orlando, Florida- Sacramento County, California- Phoenix, Arizona- Las Vegas, Nevada- or Greater Twin Cities Metropolitan Area, Minnesota, you must apply in person to change your name on your Social Security card--you cannot apply by mail.
